At Consulate Health Care, we understand that the transition to a skilled nursing center, whether it be for short-term rehabilitation services or long-term care, can be a difficult one for our patients and their families.
Each day lives are touched through the efforts of our caring and compassionate professionals, our Ambassadors of Care. Our employees are focused on meeting the needs and exceeding the expectations of our patients and families by providing dignified, compassionate, and individualized care to each patient that we have the privilege to serve.
As part of our "Journey Home" program, a significant percentage of our patients return home following a brief rehabilitative stay with us. For residents for whom living independently at home is no longer possible, we offer long-term care services in a comfortable and home-like setting.

Comprehensive skilled nursing and rehabilitation services are provided by each center's professional team of Registered Nurses,
Licensed Practical Nurses, Rehabilitation Therapists, Certified Nursing Assistants, and others.
Individualized care plans are designed to meet the specific needs of each patient and may include:
Services may vary slightly by healthcare center.
We involve the patient and his or her family to design an individualized plan of care that effectively
addresses the needs of the patient as well as those of the family.
Consulate Health Care centers offer post-acute services for those patients who no longer require an in-patient hospital stay but who are not ready to go home.
Our outcome-oriented rehabilitation therapy programs help patients rebuild the strength, function and mobility needed for a successful return home.
Comprehensive rehabilitation services are provided in-house at every Consulate Health Care center.
Rehabilitation services include Physical Therapy, Occupational Therapy and Speech Therapy.
Our rehabilitation professionals are licensed therapists who are specially trained to meet the
complex needs of patients recovering from a wide range of illnesses or injuries.
Occupational Therapy assists patients in gaining the skills necessary for independence in their social and physical environments.
Using a wide range of methods, Occupational Therapists adapt tasks and modify the environment to enhance each patient’s ability to function as independently as possible.
Speech-Language Therapists address the needs of patients with communication difficulty, as well as swallowing dysfunction, by designing treatment plans to address functional abilities and promote optimal interaction within the environment. Our Speech-Language Pathologists design a treatment plan relevant to each patient’s specific needs.
Alzheimer's disease and other types of dementia can have a devastating impact - not only for the patient but also for their family members.
For family members, caring for someone with Alzheimer's disease at home can become overwhelming and often become no longer possible as
the disease progresses. We understand both the emotional and physical challenges that come with Alzheimer’s disease and other dementias, and we’re here to help.
Many of our senior healthcare centers offer specialized care for patients with Alzheimer’s disease and other forms of dementia and memory loss.
We’re dedicated to preserving the dignity and individuality of every resident, and we develop a care plan for each resident that focuses on his or her abilities,
while addressing the obstacles associated with the disease process.
In addition to short-term rehabilitation programs for patients recovering from a hospital stay or illness or injury, each of our centers offer quality,
long-term, extended-stay elder care services for those individuals for whom living independently at home is no longer possible.
The philosophy behind our assisted living program is simple – to establish an environment for mature adults where they can enjoy life to its fullest, while still
receiving the assistance they might need. Here, they can remain as independent as possible while enjoying their privacy and peace of mind.
Our assisted living centers place special emphasis on providing an atmosphere of respect and friendship where residents can get the help they want, when
they need it. Supportive services can include assistance with daily personal care, reminders to take medications, or housekeeping.
Consulate’s friendly, healthcare team is available 24-hours a day.
